If you desire any of these services, check the corresponding box(es) A special request field has been provided at the bottom to accommodate special requests or inquiries. Pre-payment for spring services is not required. Only those services authorized by the customer on this form will be performed and invoiced accordingly. A ONE WEEK advance notice of your launch date is requested. Less than one week's prior notice or a will-call request for pickup or delivery of your boat from winter storage may NOT be honored.
